Operator Certification Application Forms

**Please Note**
All applications should be filled in online. When downloading your application, in Section D, you are required to select the category of exam you are writing (i.e. Water Treatment). For each category you select, you will receive an additional page to fill in. Failure to do so will result in an incomplete application as you will not obtain all the necessary forms.

2017 Renewal Information

Who needs to fill out and submit these forms?

These forms are required to be filled out and submitted by operators that have expiring certifications as of December 31, 2017, only.

Please only fill out the forms in the categories in which you are certified. For example, if you are only a certified Water Treatment Operator, then only fill out and send in the Water Treatment Certification Renewal 2017 form.

If you are certified in more than one category, please fill out all applicable corresponding forms. If you are a contract operator, please also fill out the Contractor Operator Experience Verification Form.

Once you have filled out the forms, signed the Code of Conduct, and attached training/course/conference completion certificates, scan and email to:

Upon successful completion of renewal, an updated certificate will be mailed to you at the mailing address we have on file (please allow 4-6 weeks for processing).
